Well we have proof that at least once Murphy's Law has killed Cavendish. (Love Tobaggan episode, manhole cover accident) so I'd say it's fairly safe to assume that he's been involved with the deaths of some people.

I don't think that Milo routinely causes deaths of Danville citizens. We already know that the series is connected to Phineas and Ferb, and in Phineas and Ferb, there's already an entire organization dedicated to putting a stop to unnatural disasters. Wouldn't they have removed the Murphy Family by now if they saw them as an actual threat? Don't tell me they don't know about all the chaos happening on the other side of town. Their resources are widespread enough that they have the freakin chupacabra on their side. While I doubt they would resort to bloodshed, I do think they would at least see to it that all murphy males are confined to live their lives in the barren deserts open oceans or perhaps another planet or pocket dimension.
Furthermore, observe how Milo usually reacts to things going horribly wrong. Usually he whips something out to put a stop to it. Granted it doesn't ALWAYS work that way, but look at all the times he's pulled it off! He has at least a 50% success rate if memory serves. Also, apparently the safety market in Danville is so ravenous, that even a penniless dweeb like Elliot was able to effortly make his OWN BRAND of safety equipment!
And let's take a look at what's apparently survivable in the Danville universe. How many times has Doctor Doofenschmirtz been blown to smithereens? Even Vanessa comments on it and how he's always fine at the end of the day. And in one future, we see Perry survive a giant ball of metal flying towards him at mad speed! Granted it puts him in a fullbody cast, but survival is survival.
Also, how about the backlash effect? In the crossover, we see that Phineas and Ferb naturally have excellent luck. But in "summer belongs to you" we see a sudden change in said luck, where everything that can go wrong does, and not a murphy in sight. So take this into account. Why did their good luck suddenly go bad? I believe that there's a sort of backlash effect for the PnF Effect, same for Murphy's Law. Observe, many of the situations Milo finds himself in, end FANTASTICALLY! He'll almost always arrive on time, win any event, and constantly make a name for himself as one of the most persevering and likeable people in Danville. And PnF, while they had amazing luck all summer, (I assume theirs is more stable because of the fact it's two people rather than one) had just ONE DAY of relatively endless failure.
Another thing to consider, is that everything that can go wrong, will. And for some people, Milo and/or his father being found guilty of a crime like manslaughter would be absolutely wonderful. Especially for… Bradley, was it? The kid with black hair and a vine for a hand? He would love to see Milo (and probably his dad) go down, down, down. But everything that can go wrong for him and his dreams of freedom, DOES. Any close call and clean shave and near death is always short of incriminating. Every opportunity for Milo to cause REAL damage is dis-satisfactory. People get injured and inconvenienced, but no one is left mourning the loss of a family member, pursuing legal action in their grievous rage. So, although the general public's wish for a quiet life remains unfulfilled, so does their wish for something decisive to happen.
Also, again, Phineas and Ferb live in this city. You think no one would come up to them all like "Hey, guys, my cousin's not doing so well at the hospital, can you make some medicine to fix him and stuff?" And then Phineas be like "Ferb, I know what we're gonna do today!" And then they go revolutionize the medical industry because screw physics they're Phineas and Ferb. I bet that the mortality rate in Danville is almost disgustingly low. If they could make the Gordian knot out of licorice and use carrot juice to make xray vision goggles, borderline necromancy should be perfectly reasonable.
Also, Danville has been enduring Inators for at least two years, and Murphy's Law for MORE than a DECADE, courtesy of Milo's dad. I think everyone there is WELL aware that it's a danger zone, and they're all a little more safety conscious than you and I.
Though I must admit, I do like the theory about Milo having hurt Bradley in some way. Though I think he'd be a hell of a lot angrier if it were his parents that got iced, so I think it was just a personal injury at a very critical time. Or maybe he's had a crush on Melissa this whole time and he's jealous of Milo for always hanging out with her. I mean, she has a SHRINE to him in her basement or something. Granted she was trying to figure out how to break Murphy's Law, but even after she gave it up, she kept the shrine around and even kept working on it iirc.
Now, as for Milo's dad, I bet HE'S actually killed people on accident. Notice how much more nervous and insecure he is than Milo. Sometimes he outright runs off crying when something bad happens and people realize he's the cause. Phineas and Ferb weren't around during his time either, and he doesn't have a special backpack of holding. Gosh… looking back on it now, his story is so much sadder. At least he got a family that deeply and truly loves him in the end though. I'll bet that does a lot in way of soothing his guilt. And besides, he found a way to make Murphy's Law work for him, so I think he's a little more comfortable than he could have been.
Grandpa Murphy I doubt has ever seen anyone hurt by his law in a life-ending manner. He's a farmer by the look of it, so for him, it's just the cows, the pigs, and the fields of corn. In fact, Murphy's Law is probably EXACTLY why he (or his father before him who passed it on down) chose that lifestyle. No one around to hurt except the people who would know what to expect. That's kinda beautiful.
